Before Your Appointment

- Complete the owner and dog information before your appointment.
- Do not complete areas designated for the veterinarian.
- Have your dog’s registration and microchip information available.
Scheduling Your Appointment
Once the judging schedule is available, please complete the appropriate OFA form and email it to us before scheduling your appointment.
Your appointment will be scheduled after we receive your completed form.
